2021年春に大きな眼科手術器械の更新があり、Ngenuity®によるヘッズアップ手術システム、EnFocus®による術中OCT 観察装置、加えてVerion®やActive Sentry®など白内障手術機器が導入された。当科が専門とする前眼部手術における有用性を探るとともに、新たな臨床研究もいくつかスタートした。

The surgical set-up here at our hospital has recently undergone a major update. This has meant the acquisition of a range of cutting-edge equipment, including a ‘heads-up’ surgical system (Ngenuity®) and intraoperative OCT system (En Focus®), as well as new instruments for use in cataract surgery (Verion® and Active Sentry®). This has enabled us to commence fresh clinical studies focusing on the use of these technologies in anterior segment surgery.

多くのスタッフが昨年導入したDMEK に続いて、今年もいろいろな術式が導入された。術中OCT 装置を用いた角膜内皮移植、フィブリン糊を用いた翼状片無縫合手術、角膜神経移植術はその一例である。スタッフ全員が前眼部手術を専門としているため、新しい知見がすぐに共有されていくことができるのが歯科大眼科のいいところである。

Continuing a trend established last year, we have now introduced a range of new anterior segment surgeries. The insights gained from this work have been eagerly shared among the professionals who constitute our staff. As a result, we are now performing DSAEK and DMEK using an intraoperative OCT system, pterygium surgery with fibrin glue, and corneal neurotization.

コロナ禍も2年目を迎え、われわれの対処法も進化してきた。感染対策はしっかりと行いつつ、外来手術の推進や入院期間の短縮、クリニックとの連携の強化により、角膜移植を始めとする手術件数はかなり回復した。一方で、国内ドナー数の伸び悩みなど現在まで影響を受けている部門もあり、更なる工夫が求められている。

Our approach to our work has had to evolve over the past couple of years to accommodate the demands of the COVID pandemic. This evolution has enabled us to increase the surgical volume accounted for in 2021, however. This was achieved by the wider application of day surgery, shortening of hospitalization time, and cooperation with related clinics. On the other hand, some activities, such as the harvesting of corneas from domestic donors, have seen a downturn. Nonetheless, we shall continue to search for new ways to cope with such negative pressures.

数年前よりアイバンク独立の動きが進められている。2021年9月に慶大眼球銀行の業務が㈶視覚健康財団に移管されたことを受け、われわれのアイバンクも同財団との合流を目指している。本プロジェクトは、角膜センターアイバンクのみならず、眼科、市川病院、待機患者などへの影響が大きいが、2022年には更に大きな前進を果たしたいと考えている。

Here at the eye bank we have commenced moving towards greater independence. The Keio Eye Bank transferred their activities to the Vision and Health Foundation in September 2021, and our eye bank plans to do the same. While many challenges remain to be overcome, we shall continue to move forward in pursuit of major achievements in the year 2022.

白内障屈折矯正手術と緑内障を専門とする、いわば二刀流の太田友香医師が講師に昇任した。どちらも加齢に伴いニーズが増える分野でありながら、それぞれ独立した専門医師が担当している。両分野の最新知識をもった医師は国内外でも希少なので、今後の臨床や研究への活躍が期待される。

Dr. Yuka Ota, who specializes in both cataract refractive surgery and glaucoma, a double specialist so to speak, has been promoted to the position of assistant professor. Both cataract and glaucoma increase with aging, but each is usually handled by an independent specialist. Ophthalmologists with a deep knowledge of both fields are rare not just in Japan, but worldwide. Therefore, the future looks promising for Dr. Ota’s clinical and research activities in these fields.

国内承認を得た多焦点眼内レンズは10 種類以上に増え、患者様のライフスタイルによって最も適したレンズを選択できる時代になった。白内障手術後に、できるだけ眼鏡を使用しない生活を望む患者様が多焦点眼内レンズに興味をもち、水道橋病院を受診する率は増え続けている。

More than 10 types of multifocal intraocular lens have now been approved for use in Japan. This means that patients can now choose the most suitable type of lens according to their lifestyle. Multifocal intraocular lenses offer a great alternative to patients who wish to live a life without glasses after cataract surgery, and we continue to see an increase in the rate of such patients visiting our Suidobashi Hospital.
